The woman, wearing a white dress, and the elderly man got into an argument on the southbound 2 train around 11:45 a.m. on Aug. 19, police said.
As the train pulled into the W. 42nd St. station, the woman slashed the man with her pointy high-heeled shoe and then fled, police said.
The man was treated for a laceration to his head at Lenox Hill Hospital and later released, cops said.
Police described the suspect as a 5-foot-7 black woman who is around 25 years old. She weighs approximately 120 pounds and has brown eyes and long black hair.
They released a picture of the attacker and ask anyone with information to call Crime Stoppers at (800) 577 TIPS.
